Contract renewalof the hospice inpatient service to provide specialist palliative care for patients with complex end of life needs, who require 24/7 assessment and management by a specialist multidisciplinary team.
The provider is widely recognised as a pioneer of palliative care services and a leader in the field in research, innovation of service delivery and partnership working. Service delivery is built on exceptional governance and business continuity systems, underpinned by robust policies, procedures and structures ensuring consistently high standards in quality and reliability of patient care.
This service will set out to fulfill the specialist palliative care services function for patients and carers as part of a care pathway for end of life care, made up of many diverse elements of service.
End of Life care can be defined as the health and social care received in the period preceding and after death provided to both patients and their carers. It is not disease specific and covers patients with increasing general frailty at the end of their life. It is also not defined by a period of time, rather the identification of increasing need for support, within the context of progressive advanced disease. Within the over arching service delivery for end of life care, there is a requirement for a specialist element of care, and this contract continuation provides such a specialist service.
The service will address the specialist palliative care needs of the most complex patients and provide support to them as inpatients at the Hospice to maintain and improve their functional abilities and well being.
The Contract will be awarded for a 5 year period on a 3 plus 2 year extension option
