Hampshire and Isle of Wight ICB would like to invite tenders for the provision of a Community Wellbeing Service.
This proactive prevention provision will aim to meet the needs of adults who are at risk of and/or who have multiple long-term conditions/health and care needs and who may not be engaging with other health services.

two.2.4) Description of the procurement
This procurement will identify a Bidder who will deliver a Community Wellbeing Service.
This proactive prevention provision will aim to meet the needs of adults who are at risk of and/or who have multiple long-term conditions/health and care needs and who may not be engaging with other health services.
The service will commence 1st October 2024 for 3 years with the optional extension period of 2 x 24 months. Maximum budget £900,000 per annum, £6,300,000 for the total 7 years length.
The opportunity to bid is open until 4th April 2024 at 14:00 hours.
The HIOW ICB Strategy lays out five main priorities, one of which is the focus on 'Good health and proactive care.'
With an ever-increasing ageing population and the gap between health span and life span widening, focussing on a proactive and preventative approach to health care is essential if we are to tackle some of the challenges health and care systems are experiencing.
The strategy includes aims around:
• Providing proactive, joined up care for people with complex needs, including frailty.
• Supporting healthy ageing and supporting people living with the impact of ageing
• Joining up resources for greater impact on housing, mental health, refugees, homeless, rough sleepers and supporting more deprived communities

8.Click on the "View Details" button for relevant opportunity to access the content, clicking "Express an Interest" where required to do so.
9.Click the relevant procurement as required then click the "Opt-In" at the bottom of the page to start building your submission to the procurement stage. This will move the project over to your "My Tenders" page. (This is a secure area reserved for your projects only).
10.Note the deadline for completion and follow the on screen instructions to complete the RFI/SQ/ITT, accessing any attached documents as required.
