Lincolnshire’s High Needs Strategy outlines Lincolnshire County Council’s (the ‘Customer’) intention to ensure CWD have the right support at the right time to help maintain their quality of life, access high quality education, remain living at home with their family, and access their local communities so they can go on to live as independently as possible.
The Service will consist of three countywide offers of support for CWD which will consist of Early Support Learning Provision, Targeted Positive Activities and Domiciliary Care. The Service includes all three elements to provide an integrated and holistic package of support for CWD and their families, where staff will be encouraged to work across more than one element of the Service. The Service will be delivered from appropriate venues and locations, including the CWD’s own home, dependent on the element of the service being supplied.
The Service will deliver and promote social and learning opportunities for children as well as offering parents/carers a suitable break from their caring responsibilities.
