The provision of a cohesive and agile 0-19 Public Health Nursing Service comprising Health Visiting and School Nursing for children and families covering .
• Public Health Nursing including the 4-5-6 service model from the antenatal period up to 19-years and transitional support for young people up to 25-years with special educational needs and disabilities (SEND).
• Public health nursing (both health visitor and school health teams) aligned with Early Help Localities to ensure multidisciplinary working and their active participation in “Team Around the Family” and “Team Around the School” approaches
• Support for all children, young people and their families at four service levels: community, universal, universal plus and universal partnership plus
• Early identification of need, screening, developmental health reviews, and a focusing on high impact areas.
• The provision of the National Child Measurement Programme (NCMP).
• The development of a Healthy Weight Management pathway
• Health improvement with a greater focus on prevention.
• Implementation of the Maternal Early Childhood Sustained Home‐visiting (MECSH) programme to all eligible families in Hammersmith and Fulham.
