The 0-19 (25 SEND) Health and Wellbeing Service will focus on addressing health inequalities and improving the health of the most vulnerable children, young people and their families/carers in North Lincolnshire. The service will adopt a One Family Approach, working with partners in an integrated way that builds resilience, enables self-help and equality of opportunity to take part in the community offer, to help improve outcomes for children, young people, and families.

The service will lead the delivery of the Healthy Child Programme (HCP) whilst working in partnership with all services supporting children, young people and their families and ensure the early identification of children, young people and their families who are at risk of poor outcomes. The service will work in partnership with families in the greatest need to support them to improve their future health and wellbeing.
