The Integrated Social Prescribing and Community Navigation service will combine a social prescribing model with community outreach. The service aims to improve health and wellbeing for adults in City and Hackney, and reduce local inequalities in health and wellbeing outcomes, which have been exacerbated by the COVID-19 pandemic.

In line with the Health and Care Bill it is envisaged that CCGs will be abolished and replaced by Integrated Care Boards (ICBs). Any reference to CCGs as Contracting Authorities in this procurement should be interpreted as referring to the ICBs that will take their place in due course.

two.2.4) Description of the procurement

The Integrated Social Prescribing and Community Navigation service will combine a social prescribing model with community outreach. The contract shall be for a period of three years with the option for annual extensions up to a further two years. (3+2)

Total value of 5 year contract is £1,370,000. The total value is based on an annual value of £274,000 per year.
