Community Stroke Services - Sandwell
The Stroke Support Service must provide information, emotional support and practical help to the stroke client, their families, and carers for a period of 12 months post stroke through 6 weeks, 6 months and 12 months reviews. The Provider must work in the hospital and the community alongside a multidisciplinary team of professionals across health and social care to provide information and advocacy support for stroke clients and their families/carers as they work towards adjusting to the change in circumstances caused by stroke.
The ICB intends to award this contract under Direct Award Process C.
