Hartlepool Borough Council wishes to invite suitably registered care providers to tender to provide care and support services within four (4) extra care schemes in Hartlepool.
The aim of extra care housing in Hartlepool is to provide housing to enable older people to improve or maintain their independence for as long as possible. The underpinning ethos is to maintain and improve a person’s ability to remain independent within their own property and where possible to provide a home for life. Extra care in Hartlepool includes access to on site care and support services 24/7, 365 days per year to help to maintain independence.
The provider will be expected to show a commitment to partnership working, quality assurance and continuous improvement. Performance will be monitored through self-assessment and the departments monitoring process for domiciliary care providers.
