*THIS ADVERT IS FOR A MARKET ENGAGEMENT EVENT*
NHS Nottingham and Nottinghamshire Integrated Care Board (ICB) are seeking to commission a Care Navigation Service. The service will focus on the identification and management of vulnerable patients (initially those with moderate and severe frailty and subsequently those living with long term conditions). The Care Navigation Service will undertake the daily operational Multidisciplinary Team (MDT) coordination using the newly formed workflows to help with identification and management of patients. The function has a key role within the Primary Care Network, and is the interface between patients and their carers, primary care, community services, secondary care, social care, and the voluntary sector. The Service will be available to all patients aged 18 and over, registered with a GP practice across Nottingham and Nottinghamshire Integrated Care System.
This notice intends to identify providers who would like to express an interest in providing the above service and to invite all interested parties to attend a Market Engagement Event with NHS Nottingham and Nottinghamshire ICB. The contract value and duration information will be covered as part of the market engagement presentation pack.

Once registered, providers will have the option to give feedback about the draft Service Specification and also have the opportunity to provide comments on topics including mobilisation periods and any perceived issues or challenges. The deadline for providing this feedback is 12pm on the 17th February 2025. Please note, this is a draft Service Specification which may be subject to changes.
Bidders who have expressed an interest in this soft market testing exercise will have the opportunity to submit a separate response to the invitation to tender (ITT) once published. Please note this is a market testing exercise and does not commit the ICB to carry out a procurement process.
