seven.1.4) Description of the procurement:

The contract will retain the original services which includes support relating to complex obesity, offering an integrated pathway for weight related medications and surgical treatment options and this will be enhanced with the following.

The Services to be included within the Contract Modification are outlined in the Service Specification relating to 'Enabling Access to Medical and Surgical Weight Management Intervention's including the urgent pathway for high BMI patients needing Gynaecological Cancer surgery.

The provider will offer both medical and surgical interventions via an integrated pathway, collaborate with other weight management providers within the wider weight management pathway, establishing a weight management single point of access to triage all weight management referrals.

The provider has their own ITU in line with NICE requirements for bariatric surgery.

The modifications to the contract includes:

•additional requirement around establishing an integrated single point of access,

•enabling access to NICE approved weight related medication (Tirzepatide) and NHS England delivery models, which will likely incur additional demand (TBD)

•repatriation of patients from an out of area provider, who are awaiting tier 3 weight management services and choose to be treated locally

•potential modifications relating to phlebotomy arrangements may be included within the scope of the specification and contract.

The contract modification will be added to main Acute contract with an annual break clause and will be reviewed annual basis.

The specialist obesity service may be subject to change due to elements such as, ITU arrangements and wider NHSE commissioning frameworks relating to weight related medications.

National frameworks have indicated a 3-year review point to re-assess delivery options which will include e.g. access to Tirzepatide.

seven.2) Information about modifications

seven.2.1) Description of the modifications

Nature and extent of the modifications (with indication of possible earlier changes to the contract):

National frameworks have indicated a 3-year review point to re-assess delivery options which will include e.g. access to Tirzepatide.

seven.2.2) Reasons for modification

Need for additional works, services or supplies by the original contractor/concessionaire.

Description of the economic or technical reasons and the inconvenience or duplication of cost preventing a change of contractor:

The NICE "Obesity: Identification, Assessment and Management" guideline (CG189), highlights the importance of and recommends moving beyond a tiered approach to obesity care. In line with this, SNEE's new integrated weight management and complex obesity disease pathway ensures that patients receive the appropriate support from the right level of service, according to their needs.

The provider will offer an integrated service model, collaborating with a range of weight management service providers to deliver both pre- and post-complex obesity care, and leads assessment, clinical prioritisation and triage for the wider weight management pathway. This model also ensures an integrated pathway across medical and surgical interventions.

There remains a recognised incidence of serious postoperative complications. Consequently, there is a requirement for critical-care resources for the occasional post-bariatric surgery patient with direct access to an on-site Intensive Treatment Unit (ITU).

seven.2.3) Increase in price

Updated total contract value before the modifications (taking into account possible earlier contract modifications, price adaptions and average inflation)

Value excluding VAT: £1,682,486,040

Total contract value after the modifications

Value excluding VAT: £1,690,482,576
