Quality & Innovation; 20%; The lifestyle navigators, as part of the Active Dorset delivery model, are qualified in various healthcare roles. The service provided is innovative and offers flexibility to accommodate any changes in requirements, as determined through data analysis.
Value, 20% - Overall benefits provided such as enhancing accessibility to services and mitigating health disparities. The emphasis on personalised care and access to prevention-focused interventions ensures value is delivered to meet individual needs effectively. Moreover, the strategic role of Active Dorset in leading and directing individuals to local services and clubs further underlines their value proposition.
Integration, collaboration, and service sustainability , 20% - The Active Dorset team are already well integrated with our outpatient teams, a data sharing MOU is in existance to enable us to capture and evidence future outcomes. Two objectives that support service sustainability include training staff to confidently signpost patients to lifestyle services and increase overall workforce awareness of prevention focussed initiatives.
Active Dorset to recruit placement students to support with the delivery of the service at South Walks House.

The primary objective of this agreement is to address health disparities by capturing and analysing lifestyle data to identify and mitigate disparities in health outcomes.
