two.1.4) Short description

Hartlepool Borough Council is seeking submissions from suitably experienced organisations to provide rehabilitation and transitional care in a residential care home. The care home where the service will be provided must be registered with the Care Quality Commission to provide the service and be rated “Good” or above.

The contract will be a block contract for 20 beds in one unit in one care home. One of the beds will be reserved for people being discharged from hospital under a virtual ward* or for people in the community on a virtual ward* who need further oversight, cannot remain at home but do not require a hospital admission.

The successful provider will work in partnership with the Community Integrated Intermediate Care team which is comprised of therapy and nursing staff together with Community Mental Health Teams, Rehabilitation Services, Rapid Response Nursing, Care Managers, Accident & Emergency, Physiotherapists, Occupational Therapists, Reablement Team, care staff and Community Nursing.
