People living with learning disabilities (LD) face some of the greatest health inequality gaps in England.

On average men with LD die 23 years earlier than men without a LD and for women it’s 27 years earlier. Poorer physical health is for the majority of people due to preventable illnesses and in part due to physical health needs being overlooked.

The Service will provide targeted outreach to support and empower people with learning disabilities to get their learning disability annual health check, to understand their health action plan, and to access vaccinations, in particular flu, pneumonia and covid vaccinations.

The commissioned service will:

Increase the number of people who been supported to receive their LD annual health check.

Increase the number of people supported to understand their personalised health action plan

Increase the number of people who been supported to receive their covid vaccination

Increase the number of people who been supported to receive their flu vaccination

Increase the number of people who been supported to receive their Pneumonia vaccination

Increase the number of GP practices engaged in the project and reporting positively about the impact of the project.

Be perceived as a positive experience by people with learning disabilities.
