The aim of this service is to provide flexible floating support to individuals, 16 years old and above, transitioning into independent accommodation or living in independent accommodation. The service will ensure they are able to manage their accommodation, develop support networks and maintain their health and well being.

two.2.4) Description of the procurement
The service will offer flexible support in transition, working with people in supported accommodation or temporary forms of accommodation who are awaiting an independent tenancy and remaining with them until they are settled in accommodation.
The service will aim to develop relationships with clients and build on their strengths and aspirations to support them to manage independent accommodation.
The service will offer support to young people 16-25 and individuals with mental health support needs ensuring that support to these individuals is tailored to their individual needs.
The service will offer short term (on average 9 months) support with the ability to increase the length of support should this be required based on individual needs.
