Personalised Care in the NHS: As part of the drive towards Universal Personalised Care, (with 2.5 million people planned to receive personalised care by 2023/2024)., the Comprehensive Model for Personalised Care,(https://www.england.nhs.uk/personalisedcare/upc/comprehensive-model/ ) has been co-produced with people with lived experience and a wide range of stakeholders and brings together six evidence-based and inter-linked components, each of which is defined by a standard, replicable delivery model. The components are: 1. Shared decision making 2. Personalised care and support planning 3. Enabling choice, including legal rights to choice 4. Social prescribing and community-based support 5. Supported self-management 6. Personal health budgets and integrated personal budgets The provision of social prescribing software, related solutions and services that help to deliver NHS Long Term Plan commitments, enable the NHS to provide the highest possible quality of safe and appropriately directed services at national, regional, local system, organisational and individual site levels. NHS England and Improvement are giving notification to social prescribing service suppliers of their intention to create a social prescribing service description to sit within the Health Systems Support Framework - www.england.nhs.uk/hssf Please note that at present this notification is just for information purposes only. This notice does not represent an invitation to tender, nor is it a request for formal expressions of interest. Suppliers interested in receiving further information and future updates may however register their interest now via the Health Family eCommercial System (Atamis) at https://health-family.force.com/s/Welcome registering their interest for project C21891 (Social Prescribing - HSSF).
