Background

His Majesty's Government (HMG) works to deliver better life chances for all - breaking the link between background and success. We do this by protecting children and ensuring the delivery of higher standards of education, training and care. The variation and poor quality of the transition from children's services into ASC is a problem raised in many reports. Improving this transition is important for supporting young people with special educational needs and disabilities to get the right support and to live as independently as possible in adulthood.

We are seeking to address the cultural and join-up problems by upskilling the social care sector via a set of best practice training materials and guidelines. The guidelines should be overarching principles for good transitions, drawing on existing materials such as NICE guidelines . These materials will improve the transitions capabilities of both children and adult social workers and leaders in the sector.

The development of deliverables, and decisions as to their format, must be informed by engagement with those with lived experience of the transition into ASC (this should include young people, young carers, family, unpaid carers, relevant education settings and practitioners working in the sector).
