In particular the LTC community out-reach health and wellbeing checks, Pentathlon
healthy lifestyle courses and Expert Patient Programme (EPP) LTC self-management

to adopt healthier lifestyles to reduce the risks of LTC, support early identification and
diagnosis and self-management in key Croydon communities.
• Empower patients with LTCs to feel more confident in managing their own condition and
keep them active and engaged in their lives.
