Complex and Live in Care - Continuing Packages of Care

Description
The ICB has recently commissioned a new Complex and live in Care framework. This procurement relates to a small number of providers, not currently on this framework but who already have Devon patients under their care. Following a clinical review of these patients a contract has been awarded to ensure continuous care and ensure no detrimental impact on patients.
