Services support people to live as well as possible in their communities.<br/>The aim of the service is to provide Community mental health support for enduring mental health needs; services working as part of the community mental health transformation programme, within the Leeds Health and Care Partnership.<br/><br/>Contract will commence on 1 July 2025 with a term of 3+2 Years. The Contract value will be £7,165,875
