Services procured are as detailed in the tender documentation.\nThis is a well-established Supported Living scheme with Leicester City Council working in partnership with Platform Housing Group and the care and support provider at this site. \nThis three-storey scheme is located within Leicester City and has 16 self-contained flats and provides an opportunity to provide supported living services to eight people. The remaining rooms within Harrison Court are: 3x supported living voids, 1x communal room and the remaining flats are used by Platform Housing Group for general needs tenants. \nThe Provider will work closely with Care Management and health professionals to monitor individuals’ health and identify any deterioration in their ability to manage health conditions, promptly informing Commissioners and relevant professionals of any significant changes. Staff must understand the needs of people with physical and sensory disabilities to ensure timely, responsive, and compassionate support. They should also be knowledgeable about available services and support, such as aids and adaptations, assistive technology, hearing support, sign language services, and carer support, enabling effective signposting.
