This project seeks to utilise public health management data, mitigate health inequalities and improve accessibility through risk stratification and review utlising digital technology, of patients with moderate to severe Asthma, who would benefit from care planning and optimal guideline driven medication therapy.\nSevere asthma is a debilitating form of asthma affecting ~4% of patients with asthma. These patients have on average\n4 asthma attacks/year, 4× more hospitalisations vs patients with asthma, and often exposed to high levels of oral\nsteroids, with significant short- and long-term side effects. Patients with severe disease account for ~50% of all asthma related healthcare costs. Patients indicating their desire to participate in the asthma winter readiness service, will give their consent to be contacted by the provider. As part of the patient consent process, patients will be asked to complete an online digital patient questionnaire. Patients identified as having missing data or non-responders will also be contacted for targeted sign up and support to complete the online survey.\nStage 4 - Patient stratification \nOnce all required information is gathered, patients will automatically enter a risk stratification AI algorithm, pre-agreed by the Project Group, with defined parameters to triage patients based on their risk of future events. Patients will be stratified into high (red), medium (amber) and low (green) risk groups as outlined in section 3.2. \nStage 5 - Patient review \nThe provider, using baseline health data provided by ECLIPSE and supplementary information provided by the patient, will contact the patients and run a comprehensive review and education service.
