The service will be based on activity.\nThe London Borough of Camden ('the Authority') who are seeking to award two contracts, North and South for the provision of Learning Disabilities Community Support. The proposed Learning Disability Community Support service model aims to promote the inclusion and independence of people with Learning Disabilities in Camden and prevent, reduce and delay the demand for more restrictive social care services. It will take a strengths-base and a locality-based approach\nThe contract will be for an initial period of four years and six months commencing on 1st October 2025 with the option of one extension of three years.   \nThe Price Quality Ratio is 40:60\nTendering organisations may apply for both Lots however they will only be awarded one lot and are asked to express their preference when applying for both Lots. \nThe maximum hourly rate for both Lots is £27.50.                         "title": "Learning Disabilities Floating Support",
                        "description": "The Learning Disabilities Floating Support (LDCS) service is a locality model (covering North/South Camden) that seeks to improve outcomes for children (14+) and adults (18+) with LD and a range of complex co-occurring needs, such as mental health, autism and behaviours of concern.  It is preventative in nature, as the support prevents, reduces and delays the need for more intensive accommodation-based support. \nThe service also contributes to meeting the Council's statutory duties under the Care Act and Section 117 of the Mental Health Act. The children's element of the service, (14- 25 year), applies the learning from the 'Closer to Camden' Commissioning Strategy for Children with Complex Needs, which cites the importance of an early intervention approach in reducing the need for residential placements and improving transition outcomes for young people.\nSeven priority areas were coproduced with key stakeholders and will be central to service outcomes, they include:\n•\tStructured support that promotes independence, choice and control.\n•\tCulturally competent and person-centred support.\n•\tReducing social isolation and support to make friends through employment and shared activities\n•\tImproving health outcomes through the uptake of Annual Health Checks, cancer screening and healthy living choices, for example in addressing obesity.\n•\tSupport to improve outcomes for young people transitioning from children to adult services.\n•\tSupport that can flex and respond to the diverse needs of the people eligible for the service.\n•\tThe support to be delivered by consistent and regular staff.\n•\tTo procure two strengths based individually tailored services in the North Camden/ South Camden.\n•\tTo include a transitional element for young people aged 14 to 17 years old.\n•\tTo set an hourly rate that is realistic and competitive. \n•\tEnsure borough wide consistency in quality and delivery.\n•\tCreate workforce capacity to address the recruitment and retention challenges providers are facing causing delays in residents receiving support.   \n•\tEnable better co-ordination of referrals to create efficiencies and ensure the support remains person centred, culturally competent and strengths- based\nThere are two Lots \nCommunity Support - North\nLot 1 (North) Estimated weekly hours: 1055.\nEstimated number of residents using service 160\n2\tCommunity Support - South\nLot 2, South Estimated weekly hours: 652 .\nEstimated number of residents using the service 90.",

                    "description": "The Learning Disabilities Floating Support (LDCS) service is a locality model (covering North/South Camden) that seeks to improve outcomes for children (14+) and adults (18+) with LD and a range of complex co-occurring needs, such as mental health, autism and behaviours of concern.  It is preventative in nature, as the support prevents, reduces and delays the need for more intensive accommodation-based support. \nThe service also contributes to meeting the Council's statutory duties under the Care Act and Section 117 of the Mental Health Act. The children's element of the service, (14- 25 year), applies the learning from the 'Closer to Camden' Commissioning Strategy for Children with Complex Needs, which cites the importance of an early intervention approach in reducing the need for residential placements and improving transition outcomes for young people.\nSeven priority areas were coproduced with key stakeholders and will be central to service outcomes, they include:\n•\tStructured support that promotes independence, choice and control.\n•\tCulturally competent and person-centred support.\n•\tReducing social isolation and support to make friends through employment and shared activities\n•\tImproving health outcomes through the uptake of Annual Health Checks, cancer screening and healthy living choices, for example in addressing obesity.\n•\tSupport to improve outcomes for young people transitioning from children to adult services.\n•\tSupport that can flex and respond to the diverse needs of the people eligible for the service.\n•\tThe support to be delivered by consistent and regular staff.\n•\tTo procure two strengths based individually tailored services in the North Camden/ South Camden.\n•\tTo include a transitional element for young people aged 14 to 17 years old.\n•\tTo set an hourly rate that is realistic and competitive. \n•\tEnsure borough wide consistency in quality and delivery.\n•\tCreate workforce capacity to address the recruitment and retention challenges providers are facing causing delays in residents receiving support.   \n•\tEnable better co-ordination of referrals to create efficiencies and ensure the support remains person centred, culturally competent and strengths- based\nThere are two Lots \nCommunity Support - North\nLot 1 (North) Estimated weekly hours: 1055.\nEstimated number of residents using service 160\n2\tCommunity Support - South\nLot 2, South Estimated weekly hours: 652 .\nEstimated number of residents using the service 90.",

The service will be based on activity.\nThe London Borough of Camden ('the Authority') who are seeking to award two contracts, North and South for the provision of Learning Disabilities Community Support. The proposed Learning Disability Community Support service model aims to promote the inclusion and independence of people with Learning Disabilities in Camden and prevent, reduce and delay the demand for more restrictive social care services. It will take a strengths-base and a locality-based approach\nThe contract will be for an initial period of four years and six months commencing on 1st October 2025 with the option of one extension of three years.   \nThe Price Quality Ratio is 40:60\nTendering organisations may apply for both Lots however they will only be awarded one lot and are asked to express their preference when applying for both Lots. \nThe maximum hourly rate for both Lots is £27.50. Tenderers that submit an hourly rate in excess of this may be disqualified. \nThis procurement comes under Section 9 of the Procurement Act 2023 (the act) and \"Light Touch Services\" as defined in Schedule 1 to the Procurement Regulations 2024 (the Regulations) which defines which services are Light Touch. \nThe procurement will be untaken using an open procedure as described in the Procurement Act 2023 and further detailed in Section 2 Tender Guidance.                             "value": "The Learning Disabilities Floating Support (LDCS) service is a locality model (covering North/South Camden) that seeks to improve outcomes for children (14+) and adults (18+) with LD and a range of complex co-occurring needs, such as mental health, autism and behaviours of concern.  It is preventative in nature, as the support prevents, reduces and delays the need for more intensive accommodation-based support. \nThe service also contributes to meeting the Council's statutory duties under the Care Act and Section 117 of the Mental Health Act. The children's element of the service, (14- 25 year), applies the learning from the 'Closer to Camden' Commissioning Strategy for Children with Complex Needs, which cites the importance of an early intervention approach in reducing the need for residential placements and improving transition outcomes for young people.\nSeven priority areas were coproduced with key stakeholders and will be central to service outcomes, they include:\n•\tStructured support that promotes independence, choice and control.\n•\tCulturally competent and person-centred support.\n•\tReducing social isolation and support to make friends through employment and shared activities\n•\tImproving health outcomes through the uptake of Annual Health Checks, cancer screening and healthy living choices, for example in addressing obesity.\n•\tSupport to improve outcomes for young people transitioning from children to adult services.\n•\tSupport that can flex and respond to the diverse needs of the people eligible for the service.\n•\tThe support to be delivered by consistent and regular staff.\n•\tTo procure two strengths based individually tailored services in the North Camden/ South Camden.\n•\tTo include a transitional element for young people aged 14 to 17 years old.\n•\tTo set an hourly rate that is realistic and competitive. \n•\tEnsure borough wide consistency in quality and delivery.\n•\tCreate workforce capacity to address the recruitment and retention challenges providers are facing causing delays in residents receiving support.   \n•\tEnable better co-ordination of referrals to create efficiencies and ensure the support remains person centred, culturally competent and strengths- based\nThere are two Lots \nCommunity Support - North\nLot 1 (North) Estimated weekly hours: 1055.\nEstimated number of residents using service 160\n2\tCommunity Support - South\nLot 2, South Estimated weekly hours: 652 .\nEstimated number of residents using the service 90."
